The Arizona State Hospital is the only publicly owned and operated hospital located in a 93 acres campus in Phoenix, Arizona. Arizona Health Insurance

The Affordable Care Act, also known as the Obamacare Act requires all individual, including children to get insured or pay a penalty through the Obamacare Tax Penalty based on their taxable annual income and household size. The Office of Vital Records maintains all certified copies of vital records, including birth, death, fetal death and still birth certificates, marriage certificates, divorce certificates in Arizona.

Under the state/federal law, Birth certificates are confidential records maintained by the office of Vital Records. The Center furnishes the certificates to the person named on the certificate parents, son, daughter, sister, brother, grandparents, foster parents or legal representative of the person and the applicant must be 18 years of age or older to obtain the certificates. Certain legal fees are levied to obtain the certificate or additional copies, to search the certificate and also to expedite the process.

Under the state/federal law, Death certificates are confidential records maintained by the office of Vital Records. The Center furnishes the certificate to the spouse, parents, immediate family members, or legal representative of the person. The certificates may be obtained upon payment of the proper fees and the applicant must be 18 years of age or older.

All records and certificates related to marriage and divorce in Arizona are not available from the Office of Vital Records. The records are managed and maintained by the Clerk of the Superior Court in the county where the event occurred.
